A form can be as simple as a text box(search forms)or it can be a multi-page form with several input fields. Now that you have had some experience in signing up for an online account, this next assignment focuses on completing online forms. The online forms are mainly of two types:
- E-Services forms: The one that you fill online and submit online.
- Printable forms: These you can either fill online and then print a copy or download an empty form and fill it later on.

This next video is an overview of the second type of forms. Like the form in this video, many online forms do not allow you to save the information you have entered or to save the completed document to your computer; instead, you have to print the completed form in order to keep a record.
You can practice completing an Adult Upgrading Grant (AUG) form, which many of you had to complete in order to register for this course.
AUG is a financial grant that can assist students with tuition and/or fees for programs such as Academic Upgrading (also known as Adult Basic Education or ABE), Building Employment Success for Tomorrow (BEST), Employment Training & Preparation (ETP, formerly ASE), or, English Language Development (ELD).

- Fill out page 3 of the form using the following information and putting it into the correct spaces using proper spelling and grammar (e.g. upper and lower case letters). Also, click on the appropriate boxes for the remainder of the page (and don’t forget line 21, “What is your family size?”):Student Information:Student Name: Jane or Joe Alias (depending upon your gender!)

After completing page 3, go to page 4 and complete Section 4. Name of the institution is Camosun College, and the campus will be VNFC. Click on the drop-down menu of “Course Type” and select “Adult Basic Education.” Next, enter “Comp 030-BS17” in Course Name.
Proceed on to page 5 and enter “Jane Alias” or “Joe Alias” where it says “Print Name” and then select today’s date in “Date Signed”. Congratulations – you have now completed an AUG form!
Double check to make sure you have filled everything out correctly and if you have access to a printer, print your completed form and sign it, and give it to your instructor. If you don’t have access to a printer, show your completed online form to your instructor or instructional assistant.
